Jnanasrimitra (975-1025) was regarded by both Buddhists and non-Buddhists as the most important Indian philosopher of his generation. His theory of exclusion combined a philosophy of language with a theory of conceptual content to explore the nature of words and thought. Jnanasrimitra's theory informed much of the work accomplished at Vikramasila, a monastic and educational complex instrumental to the growth of Buddhism. His ideas were also passionately debated among successive Hindu and Jain philosophers. This volume marks the first English translation (...) of Jnanasrimitra's Monograph on Exclusion, a careful, critical investigation into language, perception, and conceptual awareness. Featuring the rival arguments of Buddhist and Hindu intellectuals, among other thinkers, the Monograph reflects more than half a millennium of competing claims while providing an invaluable introduction to a crucial philosopher. Lawrence J. McCrea and Parimal G. Patil familiarize the reader with the author, themes, and topics of the text and situate Jnanasrimitra's findings within his larger intellectual milieu. Contemporary medical education must train skilled and compassionate health care professionals who are rigorous in their approach to patient care and their pursuit of knowledge and solutions. Problem-based learning has been widely introduced, but there is no evidence that it leads to better outcomes than more traditional programs, and fundamental gaps in conceptual knowledge may result. Recently, emphasis has been placed on a solid grounding in underlying concepts combined with a systems-based approach, and ability to transfer information and solve problems. (...) Integrating traditional scientific and clinical disciplines with progressive and continuous assessment, may be a better means of achieving the combined aims of clinically relevant curriculum design, vertical integration of medical knowledge, and facilitation of the continuum of training. Being adaptable and flexible, cognisant of costs, and driven by evidence are key features of delivering medical education and contemporary medical practice. Educational research should lead to continuous improvement, but innovation without evaluation and attention to costs may create as many, or more, problems as are solved. (shrink)

‘The political’ is much talked about today, but its invocation in international political theory is all but entirely dismissed. Yet, moral-ethical articulations do impact theorizing about international life, albeit in a most peculiar and often concealed fashion. In this paper I investigate the modernity of sovereignty in political and international theory and explain why invocations of the moral-ethical are so forcefully liquidated from international relations theory. I examine the constitutive effects of the sovereignty imperative and explain how modern notions of (...) ethics, politics and community are delimited by the need to effect what can be made to count as the pre-political ground of will-formation. A peculiar modernist strategy delimits the imagination of the politically possible in international life. I argue that critique must attend and not dispense with a fundamental paradoxicality (which is transfigured as modernity’s very metaphysical embodiment) that structures a distinctively modern manner of reflecting on ethics and community. The paradoxicality is sovereignty. It is accommodated by a liberal politics that works to conceal a restive anxiety of the modern ‘condition’. This anxiety is in turn productive of a politics with which we must grapple as we elaborate governance in global life. (shrink)

Background Residual dried blood spots (rDBS) from newborn screening programmes represent a valuable resource for medical research, from basic sciences, through clinical to public health. In Hong Kong, there is no legislation for biobanking. Parents’ view on the retention and use of residual newborn blood samples could be cultural-specific and is important to consider for biobanking of rDBS. Objective To study the views and concerns on long-term storage and secondary use of rDBS from newborn screening programmes among Hong Kong Chinese (...) parents. Methods A mixed-method approach was used to study the views and concerns on long-term storage and secondary use of rDBS from newborn screening programmes among Hong Kong Chinese parents of children 0–3 years or expecting parents through focus groups (8 groups; 33 participants) and a survey (n = 1012, 85% mothers) designed with insights obtained from the focus groups. We used framework analysis to summarise the themes as supportive factors, concerns and critical arguments for retention and secondary use of rDBS from focus group discussion. We used multiple logistic regression to assess factors associated with support for retention and secondary use of rDBS in the survey. Results Both in focus groups and survey, majority of parents were not aware of the potential secondary use of rDBS. Overall secondary use of rDBS in medical research was well accepted by a large proportion of Hong Kong parents, even if all potential future research could not be specified in a broad consent. However parents were concerned about potential risks of biobanking rDBS including leaking of data and mis-use of genetic information. Parents wanted to be asked for permission before rDBS are stored and mainly did not accept an “opt-out” approach. The survey showed that parents born in mainland China, compared to Hong Kong born parents, had lower awareness of newborn screening but higher support in biobanking rDBS. Higher education was associated with support in rDBS biobanking only among fathers. Conclusion Long-term storage and secondary use of rDBS from newborn screening for biomedical research and a broad consent for biobanking of rDBS are generally acceptable to Hong Kong parents given their autonomy is respected and their privacy is protected, highlighting the importance of an accountable governance and a transparent access policy for rDBS biobanks. Democracy and the state are two political notions that have come under considerable duress in late modernity. This paper considers a prominent critic of both, Sheldon Wolin. The paper examines three elements that figure in Wolin's analyses of democracy and the modern state in a central way: community, memory, and the culture of history. A theorisation of these elements can illuminate what is at stake in the articulation of political conceptions that yield communal forms through the constitution of political space. (...) Wolin's analyses of democracy, the state, and modern power can be of help, first, in elucidating the political valences of the three elements themselves; second, in specifying relationships of mutuality among them; and third, in theorising what is at issue in the transposition of these elements from the domestic sphere to the international. The paper speaks mainly to the first and second concerns. A path is explored for thinking what is at stake in the third, namely, locating in and then transposing from the domestic sphere a ‘we’ which does not enjoy a precise ontology, but which is nonetheless capable of giving collaborative efforts in world-political spheres another political ground. A new way of understanding entropy as a macroscopic property is presented. This is based on the fact that heat flows from a hot body to a cold one even when the hot one is smaller and has less energy. A quantity that determines the direction of flow is shown to be the increment of heat gained divided by the absolute temperature. The same quantity is shown to determine the direction of other processes taking place in isolated systems provided that (...) q is determined by the state of the system. Entropy emerges as the potent energy of a system [Σ], the potency being determined by 1/T. This is shown to tie in with the statistical mechanical interpretation of entropy. The treatment is shorter than the traditional one based on heat engines. (shrink)

The basis of the Periodic Table is discussed. Electronic configuration recurs in only 21 out of the 32 groups. A better basis is derived by considering the highest classical valency (v) exhibited by an element and a new measure, the highest valency in carbonyl compounds (v*). This leads to a table based on the number of outer electrons possessed by an atom (N) and the number of electrons required for it to achieve an inert (noble) gas configuration (N*). Periodicity of (...) these is nearly complete. The new basis helps to settle the question of the best form of table and related issues. (shrink)

The ethical standard for informed consent is fostered within a shared decision-making process. SDM has become a recognized and needed approach in health care decision-making. Based on an ethical foundation, the approach fosters the active engagement of patients, where the clinician presents evidence-based treatment information and options and openly elicits the patient’s values and preferences. The SDM process is affected by the context in which the information exchange occurs. Rural settings are one context that impacts the delivery of health care (...) and SDM. Rural health care is significantly influenced by economic, geographical and social characteristics. Several specific distinctive features influence rural health care decision-making—poverty, access to health care, isolation, over-lapping relationships, and a shared culture. The rural context creates challenges as well as fosters opportunities for the application of SDM as a natural dynamic within the rural provider–patient relationship. To fulfill the ethical requirements of informed consent through SDM, it is necessary to understand its inherent challenges and opportunities. Therefore, rural clinicians and ethicists need to be cognizant of the impact of the rural setting on SDM and use the insights as an opportunity to achieve SDM. (shrink)

A modern version of Lewis’s theory of valency is presented. This takes account of the results of quantum–mechanical calculations on molecules. Topics covered are polar covalent bonds, hypervalency, coordinate bonds, nonintegral bonds, oxo-anions, variable valency among transition elements, and nonclassical compounds. A distinction is drawn between the valence shell of an atom and the Lewis shell. The concept of a fractional bond pair is presented.

A simple way of introducing the theory of relativity to chemistry students is presented. This is based on experimental observations of the variation in the mass of an electron with speed. Analysis of these generates the equations chemists use, and provides a basis for critical discussion of the theory.

The valencies of the lanthanides vary more than was once thought. In addition to valencies associated with a half-full shell, there are valencies associated with a quarter- and three-quarter-full shell. This can be explained on the basis of Slater’s theory of many-electron atoms. The same theory explains the variation in complexing constants in the trivalent state. Valency in metallic and organometallic compounds is also discussed.

A simple treatment of chemical equilibrium is given, based on Boltzmann’s distribution law. The results are compared with those obtained by using thermodynamics.

A statistical mechanical treatment is given of thermal contact between two systems. Reciprocal temperature emerges from this as the relative change in the number of microscopic states a macroscopic system at equilibrium ranges over, at constant volume and chemical composition, with change in internal energy. The significance of this is discussed in detail with reference to a monatomic gas and an Einstein solid.

This article presents a personal answer to the question “What is chemistry?”, set out in terms of six propositions. These cover “pure” and “applied” chemistry, different levels of description, and the broader context of chemistry.

The mole is a difficult concept. Surveys have shown that even many teachers do not have a proper understanding of it. To help to meet this problem, the SI/IUPAC formulation of the mole is carefully presented and explained. New SI proposals are also briefly discussed.

This study empirically tests a sequential mediation model that links ethical leadership with employees’ unethical behaviour. The corruption index for countries is used as the moderator, because it represents both the instrumental ethical climate and the employee displacement of responsibility embedded in society’s ethical standards. A total of 175 participants comprising 41 teams (134 dyads) across 13 countries participated in a dyadic two-wave survey. The findings show that ethical leadership has an indirect influence on the avoidance of unethical behaviour by (...) reducing the instrumental ethical climate and by negating the displacement of individuals’ responsibility. In addition, the results also show that this process is not sensitive to the countries’ corruption levels. Such findings suggest that organizations are less prone to adjust their ethical standards to the environment than is usually expected. Accordingly, ethical leaders of MNCs may be instrumental in counteracting any corruptive pressure in the social environment, and likewise, non-ethical leaders may be a contributing factor to fostering corrupt organizations in a society that otherwise values ethical principles in business. As moral injury is a still-emerging concept within the area of military mental health, prevalence estimates for moral injury and its precursor, potentially morally injurious events (PMIEs), remain unknown for many of the world’s militaries. The present study sought to estimate the prevalence of PMIEs in the Canadian Armed Forces (CAF), using data collected from CAF personnel deployed to Afghanistan, via logistic regressions controlling for relevant sociodemographic, military, and deployment characteristics. Analyses revealed that over 65% of CAF members reported exposure (...) to at least one event that would be considered a PMIE. The most commonly PMIEs individuals reported included seeing ill or injured women and children they were unable to help (48.4%), being unable to distinguish between combatants and non-combatants (43.6%), and finding themselves in a threatening situation where they were unable to respond due to the rules of engagement under which they were required to operate (35.4%). These findings provide support for both the presence of exposure to PMIEs in CAF members and the need for formal longitudinal data collection regarding PMIE exposure and moral injury development. In the case of an intellectually disabled patient, the attending physician was restricted from writing a Do-Not-Resuscitate (DNR) order. Although the rationale for this restriction was to protect the patient from an inappropriate quality of life judgment, it resulted in a worse death than the patient would have experienced had he not been disabled. Such restrictions that are intended to protect intellectually disabled patients may violate their right to equal treatment and to a dignified death.

In September 2010 at the Johns Hopkins Hospital, one of the nation's elite academic hospitals located in East Baltimore, Maryland, Paul Warren Pardus entered the facility to visit his mother, a patient. During a discussion with her doctor in a hospital hallway, Pardus became “overwhelmed” about the care and condition of his mother, pulled a handgun from his waistband, and shot the doctor in the chest. Pardus then locked himself and his mother in her room, shot and killed her, and (...) committed suicide.Dr. Gabe Kelen, a national expert on emergency preparedness and director of the Johns Hopkins emergency department, admitted several years later that this tragic event “really got us thinking in a very serious way.” Together with colleagues at the Johns Hopkins Office of Critical Event Preparedness and Response, Kelen conducted research on the threat and impact of “active shooters” in health care settings. Although recent studies of his moral thought have touched on a number of topics, the majority of these have focused on his account of the virtues and their place in the Christian life. Probing the questions of the relation of virtue and law, the role of reason and will, and the place of the passions in Aquinas's moral theology, I will examine (...) recent studies by Diana Cates, Pamela Hall, Simon Harak, James Keenan, Daniel Nelson, Daniel Westberg, and Paul Waddell.
